形状记忆合金在四肢骨折中的应用前景

[摘要] 目的 为了探索形状记忆合金在四肢骨折中的应用价值。方法 对89例四肢常见骨折采用切开复位，应用形状记忆合金内固定物进行固定，术后进行随访，观察分析其效果。结果 03.4.5-05.9.26期间共应用形状忆合金内固定物固定四肢骨折89例，其中68例获随访，随访时间2~21个月，平均11.4个月。骨愈合率达100%。手术平均用时为40.81分钟。1例肱骨干骨折术后出现桡神经损伤症状，经保守治疗5个月痊愈。1例指骨开放性骨折术后出现创口感染、内固定物外露经二期扩创、锁骨下皮瓣转移而覆盖。无内固定失效、内植物排斥反应、再骨折发生。结论 形状记忆合金在骨折内固定中有很大的优势，手术操作简便、手术时间短、固定可靠、并发症少，而且对骨干几乎不产生应力遮挡，促进骨愈合。但要注意应用的范围，明确适应证，术中防止损伤骨周重要结构。

[Abstract]Objective To explore the values of sharp memory compressive connector applied on arm and legs fractures. Methods 89 cases of arm and legs fracture patients who were fixed with sharp memory compressive connector were followed up and reviewed, results were analysesed . Results 68 cases were followed up from 2 to 16 months (the average was 9.4 months ).No cases had failed internal fixation ,reject reaction ,second fracture ,non-union .Only one case with radial-nerve damaged were found after operation. Conclusions The sharp memory compressive connector is the internal fixation materials for the treatment of arm and legs fractures due to its advantages of strong fixation ,few complications, short operating time , mechanical property of fixed bone and beneficial to fracture healing .The adapted indication of operation should be grasp strictly.

随着生物力学和生物材料学领域的飞速发展, 镍钛记忆合金近几年来成为应用于骨折内固定的新型内固定材料，按这种材料特性材料制成的环抱器、环抱接骨板或抓髌器等，具有手术方法简单、固定安全有效，又符合生物力学固定等优势，已被临床广泛应用。四肢骨折的内固定方法种类虽多，但各有利弊。治疗时应根据受伤情况、骨折类型及医师操作技术熟练程度加以选择。我院在03.4.5-05.9.26期间共应用形状忆合金内固定物固定四肢骨折89 *作者单位：321200 浙江，武义县第一人民医院骨科
